Forms (Insurance forms, Income protection etc) 

Please help the doctor by filling in the appropriate details before passing to the reception staff. It may take several days to complete these forms and there is normally a charge. 


Fit Notes

You only need a Fit Note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.  

After having had a Fit note, many patients ask, ‘Do I need a note saying I’m for for work?’ ‘No’, you do not need to see your GP again to be signed back to work. Unlike a ‘sick line’, the ‘fit note’ does not have an option to say that you are fit for work. If your GP wants to assess your fitness for work again, they will state this on your fit note.


Emergency Dental Care

For those registered with a dentist please always contact him/her for dental problems such as pain or possible abscess. Patients who are not registered with a dentist should contact the Lothian Dental Advice Line on 0131 536 4800 Monday-Friday 8.00am to 6.00pm. NHS 24 will provide advice out with normal hours on 111.


Non-NHS Medical Exams 

Examinations for special purposes (e.g. insurance, HGV, PSV) do not come under the NHS. There is a fee for this service as recommended by the British Medical Association. 


Disabled Access

We have full access for disabled patients via a ramp next to the steps at the main entrance and there are no steps within the health centre building. 


Information Leaflets

Various information leaflets on a variety of topics and health promotion materials are available in the surgery. Most of the leaflets are displayed in the Waiting Room but if you require further information please ask at reception.
