Registering With Us


Thank you for enquiring regarding our Registration process.  Our reception staff are happy to help you register with the Practice, however to do this they will need to ascertain if you are staying permanently in the Practice area. We are happy to welcome patients from  Craigshill, Mid Calder, Pumpherston and Uphall Station.  We unfortunately cannot care for patients who are outside this area. Sadly this also includes existing patients who move outside the area - our area is comparatively very large but we have to set limits, especially as we may need to visit you at home. You may be asked to register elsewhere if you are out with our catchment area.


You will be given a patient questionnaire and registration form which needs completed at the time of registration as we are unable to make an appointment for you until this has been done. Proof of address will be required in order to register with our practice. NHS Lothian have provided an 'Information for Patients' sheet, which details which items of identification are required before you can register with us.  'Information for Patients'.


You will be registered with the Practice, not with an individual GP. This does not prevent you seeing the GP of your choice, however, this may result in you having to wait a little longer.


Once all forms have been completed and you have provided the necessary identification, please return all of these to our receptionist who will make you an appointment in the first instance with the practice nurse for a new patient medical.


Alternatively, please complete the forms 'Registration Form' and 'New Patient Questionnaire'.  Once you have completed these, please bring them down to the surgery. New Patient Registration Forms must be completed by each person registering with us. Patient Quesionnaires need only be completed for those over the age of five.


PLEASE REMEMBER: you are not registered with the practice until you have come into the Health Centre and produced the required identification and you have signed the registration form.
